PDF for Academic Record and High School Profile

National Merit Scholarship Corporation (NMSC) is making available to high schools this Portable Document Format (PDF) option for completing the sections of the application related to the applicant's academic record and the school's grading system and curriculum. The PDF permits the user to fill in, save, edit, and print the information.

NOTE: This PDF option is only intended for high schools choosing to use the paper application. If your high school has elected to use the Online Scholarship Application (OSA), please refer to the letter or email you received with your temporary login credentials for details about logging in to the OSA.
