Forms for High Schools

National Merit Scholarship Corporation (NMSC) is making available to high schools this Portable Document Format (PDF) option for completing the sections of the application related to the candidate's academic record and the school's grading system and curriculum. The PDF permits the user to fill in, save, edit, and print the forms.

To use this option, Adobe Acrobat Reader, version 8.0 or later, is required.
